Step 1: Online Registration Student and parent must create an account on www.planeths.com and complete all forms Registration instructions can be found by CLICKING HERE
Step 2: Pre-participation Physical Schedule an appointment with your physician to complete a physical exam. Sports physicals will only be accepted on New Jersey Department of Education Pre-Participation Forms (5 pages)

Step 3: Submit Completed Physical Upload the completed sports physical Medical Eligibilty Form to the student account by logging in to www.planeths.com |
Any student with current or history of asthma or allergies must have an action plan on file updated every school year. Please take the applicable forms with you to your physical appointment.
IF YOU HAVE HAD A PHYSICAL IN THE PAST YEAR BUT HAVE NOT TURNED ONE IN TO THE ATHLETIC OFFICE, PLEASE HAVE YOUR DOCTOR COMPLETE THE PHYSICAL FORMS AND UPLOAD TO YOUR PLANETHS.COM ACCOUNT.
